This is my application for joining Personal Assistants at Home Membership Network. I confirm that:

1. I am willing to have a DBS (Criminal Records) check 

2. I will supply two references from former clients, people I know, members of the educational establishment I attend ro such other persons as I may agree with Personal Assistants at Home

3. I agree to carry appropriate insurance for my work and also for any vehicle I may drive for my business.

4. I understand that on acceptance after background checks have been completed the cost to be a member is £3.50 a week which I am willing to pay.

5.I understand that I must agree to abide by the Code of Conduct for Personal Assistants.

I understand that on acceptance as a Member I will have access to the following benefits:
